Primary Care (Prime Care)

Mission Statement/Description of Service

Primary Care strives to provide the Veteran with timely access to health care. Our employees promise is to treat you with courtesy and dignity. We will provide information and education about your health care in a forum that you can understand. We endeavor to provide support to meet your emotional needs, while providing opportunities to involve your loved ones in your care. Prime Care provides treatment in a compassionate and patient focused environment.

Primary Care offers comprehensive outpatient health services for Veterans in West Central Ohio and Eastern Indiana through the Dayton Prime Care Clinic and four Community-Based Outpatient Clinics located in Middletown, Lima, Springfield, and Richmond, Indiana. In addition, our Emergency Department is staffed 24 hours/day, seven days/week. As a service for Veterans with chronic disease, the Chronic Disease Care Team, consisting of a physician, advanced practice nurses, dieticians and pharmacists, is available to provide specialized treatment to minimize the consequences of several chronic diseases. Primary Care also conducts community and suicide prevention outreach with a state-of-the-art mobile van, staffed with a team of Primary Care medical professionals. Lastly, Home-Based Prime Care services are available to those Veterans with difficulty getting out of the home.
